New Patients
Before Your First Visit
A little preparation helps your first appointment go smoothly. Please bring:
- A photo ID
- Your dental insurance card, if you have one
- Your completed New Patient Form
- A list of any medications you take
- Plan to arrive 10–15 minutes early

Print it, fill it out at home, and bring it with you. If you can’t print it, no problem. We’ll have a copy ready for you at the front desk.

New patients without insurance get an exam, X-rays and a regular cleaning for $99, plus a free electric toothbrush for every new patient.

When should my child first see the dentist?
We recommend your child’s first visit by age 1. Early visits help kids feel comfortable at the dentist and let us catch any problems early.
